Our Facility

Located at 101 Noons Creek Drive in beautiful Port Moody, our 10-bed Hospice is run in partnership with Fraser Health. Our Hospice allows individuals to live their final days with peace and comfort in a home-like setting where family, friends and pets are welcome.​

​

Each of our ten private bedrooms is fully furnished and provides the option for family members to stay overnight. Only steps from each bedroom is our Great Room, a living and dining area for loved ones to gather together to enjoy a meal, spend time with a puzzle, or get crafty.

 

While in the Great Room, the air will be filled with the aroma of home-cooked meals being prepared for patients, and baked goods for families and friends.

 

Our Hospice also has two beautiful rooftop gardens, maintained with love and care by our amazing Volunteers.​

Our Care Team

Several people make up the Care Team at Crossroads Hospice. The Care Team is comprised of Fraser Health staff, Crossroads Hospice Society staff, and Hospice Society volunteers. Fraser Health is responsible for the medical and nursing care of all patients and provides a team of doctors, nurses, patient care coordinator, social worker, spiritual care coordinator, and pharmacist.

Crossroads Hospice Society is responsible for facility management and provides the non-medical support and services including kitchen and housekeeping operations.

 

The Society also provides emotional and psychosocial support for patients and their families, in addition to complimentary programs.

Admission Process

Individuals must be receiving Fraser Health (FH) Palliative Care Services through a referral from a physician or health care professional. The Palliative Care Team will conduct an assessment and determine eligibility for palliative care services including admission into Hospice.

Cost of Care in Hospice
​

As with other residential care facilities, patients pay a daily charge for the days they receive care. The fee is the lowest rate as established each year by the BC Ministry of Health.

​

Bills are sent out at the end of a patient’s stay or the end of each month to the person identified as responsible for financial affairs.
Per diem payments may be made:

​

  • By cash, cheque or credit card

  • Through the mail to the address provided on bill

  • Over the phone (using Visa or Mastercard)

    If you are between the ages of 13 and 29, living in the Tri-Cities (Coquitlam, Port Coquitlam, Port Moody, Anmore or Belcarra) and have experienced the loss of a loved one within the last three years you are eligible to receive support services.
  • How do I access services?
    If you are between the ages of 13 and 29, living in the Tri-Cities (Coquitlam, Port Coquitlam, Port Moody, Anmore or Belcarra) and have experienced the loss of a loved one within the last three years you are eligible to receive support services.   • What is the cost?
    There is no cost to our grief support programs and services.

  • I know a grieving youth or young adult - how do I get them support?
    If you are seeking support for a teenager between the ages of 13 – 18, our Youth and Young Adult Bereavement Coordinator would love to chat with you to learn more about them and their experience with grief. You can call our coordinator at 604-945-0606 ext. 2 or email youth@crossroadshospice.org to arrange a time to connect. To ensure privacy and confidentiality, anyone between the ages of 19 and 29 must contact Crossroads on their own. We would love for you to pass along our information to anyone in your life that you feel would benefit from grief support!
  • Do you provide grief counselling?
    We offer support specializing in grief and loss, however we are not registered counsellors. At Crossroads we acknowledge that grief is a natural, but incredibly difficult, part of life. Our role is to offer emotional support and a safe place to explore grief. We also make referrals to specialized professionals if further support is needed.
  • How long do I have to wait after my person has died to contact you?
    There is not a certain amount of time that has to pass before you can reach out. However, it’s important to note we don’t provide crisis support. When someone dies, the emotions that arise are so overwhelming, it can leave you feeling terrified. Know this is not uncommon! The first few weeks after a death are usually a whirlwind, so we often will suggest waiting until these weeks have passed before making an appointment. This ensures you can give the necessary attention, love and energy to yourself to begin to explore your grief.
  • What should I expect?
    The process of accessing our programs and services are as follows: ​ Our Bereavement Coordinator will respond to you within 2 – 3 business days. An initial meeting with our Bereavement Coordinator is the first step to receiving support. The initial meeting is really just to get to learn about you and your grief experience this far. There is never any pressure to disclose more than you are comfortable with. All our program and services are provided by our Bereavement Coordinators, as well as specially trained and experienced Volunteers.
  • Do you provide grief support for non-death losses, like divorce or job loss?
    Though many life events can have a profound impact on well-being, at Crossroads our services are specifically for grief resulting from the death of a person.
